Creatine for Night Shift Workers: Could It Help When You’re Sleep Deprived?


Could creatine monohydrate help night-shift workers maintain cognitive performance when they’re tired and sleep-deprived?

Creatine is best known for its role in strength, muscle and exercise performance. But a growing area of research is examining something very different: creatine, the brain, and sleep deprivation.

And this could have implications for people who work through the night.

In this episode of A Healthy Shift, I look at new research investigating whether creatine supplementation can reduce some of the decline in cognitive performance that occurs during sleep deprivation.

A 2026 randomised, double-blind crossover study put healthy adults through 21 hours of sleep deprivation and found that a single dose of creatine monohydrate reduced deterioration in several measures of cognitive performance.

But does that mean shift workers should start taking creatine before night shift?

Not so fast.

I break down what the research actually shows, where the evidence is still lacking and why we cannot assume that results from experimentally sleep-deprived healthy adults will automatically apply to nurses, paramedics, police, emergency services, transport workers and others working night shift.

The evidence around creatine and sleep deprivation is promising, but it remains preliminary.

A 2026 systematic review identified only five eligible studies examining creatine supplementation during acute sleep deprivation. While the overall findings were favourable, researchers concluded that more high-quality research is needed.

For shift workers, the question is an important one.

Could creatine monohydrate eventually become another tool for supporting cognitive performance when the sleep-deprived brain is being asked to perform during the biological night?

We don't have a definitive answer yet.

But it's a question worth investigating.

Could creatine help you get through night shift? And before you think I'm talking about building muscle or getting stronger in the gym, I'm actually not. What I'm doing is I'm talking about your brain. Because we now have research showing that creatine monohydrate may reduce some of the decline in cognitive performance that occurs when people are sleep deprived. And there's something that's particularly interesting here. The cognitive benefits of creatine in healthy, well-rested adults have generally been limited or inconsistent. But what happens when that same brain is tired, is sleep deprived, and under greater metabolic stress? Hi, shift workers, that starts to look a little bit different. And if there's one population that regularly has to think, react, make decisions, and perform while tired and working through the biological night, it's you, the shift worker. So today, what I want to do is I want to look at creatine and night shift. What does the evidence actually show? Could creatine monohydrate help shift workers? Should you be taking creatine before your next night shift? And the answer to that last question isn't as straightforward as some people on the internet might have you believe. And we're talking about this because it doesn't have to be this way. Whether you're a shift worker, trying to feel better, or a leader responsible for the people who keep your organisation running around the clock, you're in the right place. Why Creatine Research Keeps Shifting

And welcome back to a healthy shift. You are more than welcome, and I sincerely appreciate you as we go into episode number 401. That's right, we've completed 400 episodes of a Healthy Shift podcast. It's quite incredible when I look back over it. Now, today's topic is a topic I have covered a few times, but I do keep a very close eye on the research around creatine monohydrate, and I want to keep you up to date on the strategies that we can use or not, or what does the latest evidence tell us? Now, this episode came about because I have been watching the research around creatine and the brain for quite some time. It's been confusing to say the least. Now, most people know that creatine is a sports supplement. You take creatine, you train, it helps with your strength, your power, and your exercise performance. And that's absolutely where most of us first came across it. But creatine monohydrate is one of the most extensively researched nutritional supplements available. And research into creatine has moved well beyond the gym now. It's actually fascinating watching where it's going. Now, we're seeing increased interest in creatine and cognitive performance, brain health, and in particular, which is really relevant to you, the shift worker, sleep deprivation. Now, a new 2026 study has caught my attention because it adds to a small but growing body of research that suggests that creatine might become particularly useful when the brain is under stress from the lack of sleep. So naturally, my shift work brain immediately went, hang on, could this have implications for our night shift workers? And that's what I want to unpack today. But I'm going to put the brakes on before we go any further. There is currently not enough evidence for me to sit here and tell you that creatine improves night shift performance. We just don't have that evidence. The studies that I'm discussing today were predominantly conducted in healthy adults undergoing experimental sleep deprivation. They weren't nurses working four nights in a row, and they weren't paramedics at four o'clock in the morning, and they certainly weren't police officers driving home after a night shift, or factory workers working permanent nights. And that is a very, very important distinction. What we have is an interesting hypothesis which is backed by some emerging evidence. Excuse me. So let's look at what we do actually know.

Brain Energy Basics And ATP

First of all, let's talk about what creatine actually does. Because creatine is something that your body already has. We naturally produce it, and we also obtain creatine through foods, particularly meat and fish. Now, most of the body's creatine is stored in our lean tissue or our skeletal muscle. And that's why we've traditionally associated creatine supplementation with exercise and performance. But creatine is also very present in the brain. And this is where things become very relevant to our shift working community. At a basic level, and I mean very basic, your cells use something called ATP. Now, this is short for adenosine triphosphate for energy. Now, ATP is basically the immediate energy currency of the cell. The creatine-phosphorotine system helps rapidly regenerate ATP when energy demand increases. You can think of phosphoratine as part of your body's rapid energy buffering system. And your brain requires a lot of energy. Think about that. Now think about what happens when you've been awake for a long time. You're tired, your attention starts slipping, your reaction time can start to really slow down, and decision making becomes really hard. Now you might find yourself reading the same line three times. You walk into a room and then you can't remember why you're there. And every night shift worker knows that horrible period in the early hours of the morning where your brain just doesn't feel as sharp as it normally does. Now, I want to be clear, that's not a lack of motivation. There are biological reasons why you're struggling. You're awake and trying to perform at a time when your circadian system is promoting sleep. That's natural. And you've also had inadequate sleep beforehand. You're adding sleep pressure on top of that circadian challenge. So researchers have started asking a really interesting question. If sleep deprivation places the brain under greater metabolic stress, could increasing creatine availability help the brain to maintain its performance? Good question.

The 2026 Sleep Deprivation Trial

So let's look at the new 2026 creatine study. This brings us to this piece of research. In 2026, Gorgi, NIAD, and colleagues published a randomized, double-blind crossover study looking at creatine during sleep deprivation. Now, to make this clear, this is an extremely high level of research. It's randomized. So different people are picked. It's double blind. So the people who are giving the supplement or the placebo don't know which one is which, and the people that are receiving it don't know which one is which. So it's what we call a double blind crossover. And then they swapped around. They took 29 healthy, and I want to make that clear. 29 healthy adults, and then they kept them awake for 21 hours. Now this is simulated, all right? This is not an actual shift worker, remember. I've been very clear on that. And participants went through the experiment twice. On one occasion, they received a placebo, and then on the other, they received a single dose of creatine monohydrate at 0.2 grams per kilogram of body weight. The researchers then tested cognitive performance throughout the period of sleep deprivation, and they found something which was quite interesting. Creatine reduced some of the deterioration in cognitive performance which was caused by the sleep deprivation. Now we're talking about areas including logical and numerical tasks, language-related processing speed, and psychomotor vigilance. Some improvements were reported at up at about 12%. Now that might not seem much, but there was another interesting observation as well. The female participants appeared to experience greater benefit across several cognitive measures. But let's be careful with that one. Because 29 participants is nowhere near enough for me to tell female shift workers that creatine works better for women. It's an interesting finding, but that's all. It gives researchers something that's worth investigating further over time. But the broader finding is what actually interests me personally. Because when the people were sleep deprived, creatine appeared to help preserve some aspects of cognitive performance. Now, this is not the first study. This research didn't just suddenly appear out of nowhere. Back in 2006, McMorris and colleagues investigated creatine supplementation during sleep deprivation combined with mild exercise. Again, participants received creatine. But this time, researchers used a much larger single dose of creatine at weight for it, 0.35 grams per kilogram of body weight. That's significant. They weren't just measuring cognitive performance either. What they were doing was looking at the changes in the cerebral high energy phosphate metabolism, the brain. And the creatine intervention produced changes in brain energy metabolism alongside improvements in cognitive performance during sleep deprivation. Now we're starting to get somewhere really interesting because it gives us a possible mechanism. Creatine is not acting like caffeine, we're not simply stimulating the nervous system to make someone feel more awake. The hypothesis involves cellular energy availability in the brain. And that could help explain something else that we've seen in creatine research. The creatine might matter more when you're tired. Because the research into creatine and cognition in healthy and well-rested adults hasn't always shown huge benefit. There are positive findings in some cognitive domains, but overall the results have been quite mixed. And I've been thinking about this from our shift worker perspective. What if that's precisely the point? If you're well rested, well fed, and your brain's energy requirements are comfortably being met, increasing creatine availability probably won't make much of a difference. But let's put that brain under some metabolic stress. Now, things may change. Sleep deprivation creates a completely different environment. Your brain is being asked to maintain attention, reaction time, and decision making despite being awake for an extended period. And the phosphoratine system helps to maintain ATP availability when the energy demand increases. So there is a biologically plausible reason why creatine could potentially become more useful during periods of sleep deprivation. And this is where I think we need research specifically in shift workers. Hello, I say this all the time. So what does this actually mean for night shift? What This Means For Shift Workers

this podcast at two or three o'clock in the morning. You could be a nurse or a paramedic, police officer, a firefighter, you might be a radio dispatcher, truck driver, factory worker, security worker, or just any industry that's operating around the clock. You don't get to stop making decisions because your circadian rhythm thinks you should be asleep. And this is the problem. And sometimes the decisions being made at three or four in the morning have serious consequences. So could creatine supplementation help night shift workers maintain cognitive performance? Possibly. But I need to stress that word. Possibly. Here's the chain of evidence that we currently have. Night shift can result in insufficient sleep, sleepiness, and circadian disruption. No shit. We know sleep deprivation can impair cognitive performance. No shit again. And we are now we now have emerging evidence that creatine supplementation may reduce some of that cognitive deterioration during experimentally induced sleep deprivation. What we can't do is jump from those findings to creatine improves night shift performance. That is not what the research has actually demonstrated. We need properly controlled trials conducted in actual shift workers. So if you're a researcher and you're listening to that, put your hand up and have a go at doing that with shift workers. And if you need them, I'm more than happy to help find some for you. Because I have a fair following that I'm quite convinced that would be more than happy to support you with this. And there's another reason why I'm also being cautious on this. Because the 2026 Systematic Review, which is a very high level of study again, specifically looked at creatine supplementation during acute sleep deprivation. Do you know how many studies met their inclusion criteria? Five. That's right. Only five. The overall direction of the findings was in fact favorable. But the researchers themselves concluded that the evidence remains sparse and that we need more high-quality trials. So when you see someone online saying science proves creatine fixes sleep deprivation, no. No, it doesn't. The evidence is promising, but it's preliminary. And those are two very, very different things. So let's get to the actual question. Should you take creatine before night shift? Now I know what some of you are thinking, Raj, you've told me all this, but

Dose Hype And Real Limitations

how much do I take before work tonight? Not so fast. Because this is another place where social media can get research badly wrong. The latest study in 2026, and this podcast is being published in August of 2026, only used, and let's be clear, 0.2 grams of creatine monohydrate per kilogram of body weight as a single dose. Let's put this in perspective. If you weigh 80 kilograms, that is 16 grams of creatine. Now, the earlier 2024 study used 0.35 grams per kilogram, which means for an 80 kilogram person, that's actually 28 grams. Now, those are substantial acute doses. They are very different from the commonly used daily maintenance dose of around three to five grams of creatine monohydrate. And I would stand and die on this hill that every single person should be taking creatine monohydrate today, especially as a female. These studies do not establish that shift workers should start taking 16, 20, or 30 grams of creatine before a night shift. So please don't hear numbers from a scientific study and turn them into a supplementation protocol. That's not how this works. So should the shift worker consider creatine? Me personally, I think creatine monohydrate may very well be worth considering. But I want you to consider it for the right reasons. Creatine monohydrate already has extensive research history for exercise performance. It has a safety profile in healthy adults, and it is well established when it is used appropriately. And now we're seeing an interesting emergent area around brain energy, metabolism, cognition, sleep deprivation, and women's health. But for shift workers, I think that deserves attention. But creatine should never become another tool that we use to try and override sleep. This is important. You can't supplement your way out of a chronic sleep restriction. Creatine does not replace sleep. It does not correct circadian disruption. It doesn't replace daylight. It doesn't replace movement. And it certainly doesn't fix poor nutrition. And it certainly doesn't suddenly make working through the night shift harmless. But what it might eventually become is just another tool in the toolbox that we as the shift worker can use. Something that provides additional cognitive support when a sleep-deprived brain is being asked to perform under pressure. And I think that is a very, very different claim. Now, this is where I think the research needs to go next. I would love to see this research taken directly

The Research Questions That Matter

into shift working populations. Give me nurses working nights, paramedics, police, emergency services, healthcare workers, transport workers, permanent night workers, rotating shift workers, and let's investigate this properly. Does regular creatine supplementation change cognitive performance across consecutive night shifts? Does it affect psychomotor vigilance, reaction time, decision making, subjective sleepiness? Does it affect the drive home after night shift? Are there differences between men and women? Does chronic supplementation at conventional dose produce the same effect as these larger acute doses? And importantly, does creatine provide any meaningful real-world benefit after weeks, months, or years of shift work rather than one experimental sleepless night? And these are all the questions that I want to see answered. Because that's when this research becomes genuinely useful to the people who need it. So my final thoughts on this. Could creatine monohydrate help shift workers? My answer today is this. Maybe. And I know that's nowhere near as exciting as telling you that we found the next great night shift hack. But that's where the evidence currently sits. We have evidence that creatine can influence brain energy metabolism. We have several studies suggesting that creatine may reduce some cognitive deterioration during acute sleep deprivation. We have a plausible biological mechanism, and we have a population in shift workers who regularly experience sleep loss, fatigue, circadian disruption while still being required to perform optimally. This makes creatine for shift workers a fascinating area of research. But we don't yet have enough direct evidence in shift workers to make the leap from this looks promising to this works. And that's the distinction, the distinction that I really, really want you to remember. Creatine is not a substitute for sleep. It's not permission to ignore fatigue. And it's certainly not going to suddenly make your night shift biologically normal. But could creatine monohydrate eventually prove to be a useful tool for supporting the sleep-deprived shift-working brain?
